~ VACCINE REQUIREMENTS ~
Please remember to turn in documentation of any required immunizations to the office ASAP. This is a State Law for School Attendance. Kindergarten and 6th grade students have specific immunizations requirements. Those students not in compliance are subject to exclusion from school and school-related activities (sporting events, dances, etc).
The Wisconsin immunization requirements are in the link below. Parents are required to have their child vaccinated or claim a waiver by the 30th day of school.
If you have received a letter stating that your student is missing a required immunization, it is encouraged that you contact your family physician/pediatrician to schedule an appointment for your child, as soon as possible. If you are unable to get your child into the doctor, or are choosing not to vaccinate your student, the school does require a waiver to be signed.
